What is an Injury on Duty (IOD) Claim?
An Injury on Duty (IOD) claim applies when an employee is injured while performing work-related duties or develops an occupational injury that qualifies for compensation under the Compensation for Occupational Injuries and Diseases Act (COIDA).
If your claim is approved, your physiotherapy treatment may be covered through the Compensation Fund, provided all the required documentation has been submitted correctly.

What do I need to get started?
To register and process your WCA claim, we require the following documentation before treatment can proceed:
- Employer’s Report of Accident (WCL2): fully completed (Pages 1-3).
- First Medical Report (WCL4) with a full clinical description.
- Progress or Final Medical Report (WCL5), where applicable.
- Doctor’s referral letter on the doctor’s official letterhead, or on our referral form with the doctor’s signature and official practice stamp.
- Certified copy of your South African ID, issued within the last three months.
- Recent salary advice.
- Letter of employment.
Additional documentation may also be required depending on the circumstances of your injury:
- Travel Report for motor vehicle accidents (completed by your employer).
- Assault Questionnaire if your injury resulted from an assault (completed by your employer).
Providing all the required documentation from the beginning helps prevent unnecessary delays in registering your claim.
Why is complete documentation important?
The Compensation Fund cannot register or process incomplete claims.
If any required documentation is outstanding, it may delay approval of your treatment and could result in your claim becoming inactive or “stale.” In these situations, patients may become personally responsible for outstanding treatment costs until the required information has been submitted.

Important information about your claim
There are a few important requirements to keep in mind during your treatment:
- If there has been no treatment for three months, the Compensation Fund may regard treatment as completed. A new referral letter may then be required before additional physiotherapy can continue.
- If treatment is required more than two years after the original injury, approval to reopen the claim will generally be required.
- Employees of Correctional Services may also need to provide a completed G111 form before treatment can proceed.
Keeping your claim active and your documentation up to date helps ensure there are no unnecessary interruptions to your rehabilitation.
